  • St. Louis, MO-IL (1888PressRelease) September 25, 2014 - Since 2011, the National Committee for Quality Assurance (NCQA) has overseen the Recognition Program which emphasizes patient-centered care in which a primary physician becomes "home" for all care. PCMH, or Patient-Centered Medical Home, will likely become the new model of health care and reimbursements. Medical practices, such as Fremont Family Care located in Fremont, Nebraska and is a wholly owned subsidiary of Fremont Health, recently achieved Level 3 Recognition.

Level 3 is the highest Recognition a practice can achieve, with points awarded for factors such as patient access to their health record, identification and management of patient populations, practices for planning, tracking and managing patient care, and systems for reporting and measuring performance.
