The program offers customized wellness and care options, based on individual need or preferences, to address the evolving needs of today's active older adults.

    "Our tenants have peace of mind knowing they have the option of remaining in their own apartment home, to recover from or manage an illness or injury, while retaining their independence," said Mary Ann Barbato, Care Strategies' vice president. "We are able to fill the health and wellness gap with supportive services, often associated with an assisted living facility, that are within the financial means of our tenants."

    Care Strategies' on-site services include health monitoring by a registered nurse; management of chronic conditions, such as asthma and diabetes; nutrition monitoring and counseling; assistance with tasks of daily living, including personal care; housekeeping and laundry; shopping escorts; activities; coordination of medical appointments; and scheduled transportation. A registered nurse will assess and oversee each individual's health service needs and plan of care, which is developed in collaboration with the tenant, their family members and their personal physician.

Rather, they are based on need and each individual's wishes.

    Care Strategies also provides a team of competent staff including administrative oversight, certified aides and certified medication technicians who will be on-site to ensure quality care and service delivery to all participants on the program. Billing is handled directly by the company, through Medicare and Medicaid. In many instances, residents can qualify for assistance with all costs covered by Medicaid.
