  • Bergen-Passaic, NJ (1888PressRelease) October 20, 2011 - The results of a recent study, conducted by RTI International, a nonprofit research organization, analyzed patterns of care, health outcomes, and costs of care for Medicare fee-for-service (FFS) beneficiaries. The study compared the results from practices that are National Committee for Quality Assurance (NCQA)-recognized medical homes with those that are not, seeking to determine if there are particular attributes of medical homes that are more favorably related to better outcomes of care.

The study showed that patients of Patient Centered Medical Homes (PCMH) have a lower hospitalization rate, better follow-up care after hospitalization and a lower hospital readmission rate than patients at other facilities. In addition, total Medicare payments per beneficiary were 43% less to physicians associated with PCMHs.

Old Hook Medical Associates, a PCMH in Emerson NJ, scored favorably in many target areas, compared to both its benchmark and other PCMHs. "We're pleased that the results of the survey reflect the quality care provided by our practice", said Edward Gold, MD, a partner in Old Hook Medical Associates. "It reinforces our belief that PCMHs lead to better health care quality, improved involvement of patients in their own care, and a reduction of costs."

Through the Patient-Centered Medical Home at Old Hook Medical, each patient develops an ongoing relationship with a personal physician who leads a team providing for all healthcare and when needed, arranges for appropriate care with other qualified physicians. A PCMH emphasizes enhanced care through open scheduling, expanded hours and strong communication between patients, physicians, and staff.

Old Hook Medical is the only practice in Bergen County to have qualified at the third and highest level for NCQA recognition as a Patient-Centered Medical Home. In addition, the practice is already certified by the NCQA in Diabetes, and Heart/Stroke Care.

Old Hook Medical Associates is a multi-specialty healthcare practice located in Emerson, NJ, where primary care, specialty physicians, nurse practitioners and other medical professionals work together under one roof.
