The app lets users keep a cognitive behavioral therapy (CBT) thought diary entirely by voice, with AI transcription running locally on the device.

    CBT is one of the most evidence-based forms of psychotherapy, recommended by NICE, APA, and WHO for anxiety, depression, OCD, and panic disorder. Its effectiveness depends on keeping a structured thought diary between therapy sessions. In practice, most people skip this - typing during a moment of anxiety or a panic attack is not feasible.

    Mentalium removes that barrier. Users press a record button and speak through the five steps of a CBT thought record: situation, automatic thought, emotion, reaction, and alternative thought. The AI on the phone transcribes the recording and fills each field automatically. No typing is required at any point.

    The transcription model runs entirely on the device. After a one-time download of approximately 180 MB, the app works fully offline - on the subway, on a plane, or anywhere else. Diary entries stay in a local database on the phone. No voice data is sent to any server. Additional features include anxiety rating (1–10 scale) before and after each entry, charts tracking progress over time, cognitive distortion analysis across seven languages, and an encrypted Excel report that users can email to their therapist before sessions. Mentalium supports English, Russian, Spanish, French, Portuguese (Brazilian), German, and Italian. A free 7-day trial is available, followed by a subscription of $4.99 per week or $29.99 per year.
