  • (1888PressRelease) August 12, 2014 - ICD-10 brings expected and unexpected challenges. As a practice you may be still wondering how to implement ICD-10. What happens if labs are still on ICD-9 after October 1st, 2015? What happens if Lab 1 is compliant and Lab 2 is not complaint? What happens if physician office is not compliant? Lab rejects the order?. There is need for some program/solution for lab orders, radiological orders etc. HL7 is a solution.

    HL7- most widely used healthcare interoperability standard in the world in use in over 90% of hospitals in the US and widely supported by healthcare IT suppliers worldwide.

    ICD-10 Codes are carried in HL7 as CE data type. The CE data type has been withdrawn in HL7V2.6, and has been replaced by CNE and CWE.

    CNE - Coded with no exceptions

    • The first component (the code) is required.
    • The specified HL7 table or imported or externally defined coding system must be used and may not be extended with local values. Note: this means that the CNE data type cannot be used in combination with a "user-defined Table".

    CWE - Coded with Exceptions

    • The most relevant characteristics of the CWE data type (when compared with the characteristics of the CE data type) are the use of versionIDs to convey the version of the coding system, and the introduction of Original Text to convey the original text that was available to an automated process or a human before a specific code was assigned.

    So, HL7 is already prepared for ICD-10.

    If an order is received with ICD-9 codes, the HL7 tool should convert to ICD-10 codes and process the order. However medical necessity has to be met.

    The common segments are DG1, FT1, IN1, IN2, IN3, OBR, OBX, ORC, PD1, PID, PR1, PV1, PV2 etc.

    Due to granularity and specificity in the ICD-10 codes, the implementation, and GUI design is tough task. Example: Insect bite codes, pressure ulcer codes etc.
