November 06, 2009 - Right at Home, a state licensed provider of in-home care and personal assistance, is partnering with Forsyth Medical Center to assist patients 65 years and older in their home-based recovery. The Hospital to Home Program is designed to improve the quality of transitional care and increase access to services after discharge.
This partnership will focus on patients with physical disabilities, histories of falls, cognitive declines and those who have had three or more hospitalizations in one year. Right At Home will carefully match the personalities of its qualified, bonded and extensively trained caregivers who will help participants adjust between hospital discharge and a strong home-based recovery.
“Many of these patients live alone and have limited financial support,” said Greg Brewer, Agency Director. “Right at Home understands the challenges patients may face during the transition from their hospital stay to becoming independent again in their own home. We know the ability for patients to recover in their own homes has many benefits, from financial to emotional.”
The services offered through the program include a personal evaluation and recovery care management by a patient navigator, the coordination in-home services and help getting prescriptions and medical equipment. Right at Home caregivers will be available for grocery shopping, housecleaning, preparing meals and transportation to doctor’s appointments.
Operating since 2004, Right at Home has been serving the needs of the Forsyth county area with in-home care and assistance for seniors and the disabled.
