DATELINE: LAKEVILLE, MARSHFIELD AND PLYMOUTH, MA…Erica St. Germain, LCSW, has recently been named Geriatric Care Manager at C.A.R.E., LLC, Consultation, Advocacy and Resources for Elders. C.A.R.E., LLC, provides geriatric care management to elders and their families throughout the South Shore and upper Cape Cod and is headquartered in Marshfield.
In her new role at C.A.R.E., LLC, Ms. St. Germain will assist individuals and their families in finding resources and managing care of elders, which includes screening, arranging and monitoring in-home help or other services for elders. Ms. St. Germain will also provide crisis intervention (if needed) and act as a liaison between an elder, his/her family and service providers such as home health care, physicians’ offices and in and out-patient treatment centers.
Ms. St. Germain has more than 10 years of experience in the field of Elder Protective Services. Ms. St. Germain has a Bachelor of Science in Social Work from Bridgewater State College and a Master’s degree in Social Work from Simmons College. She is affiliated with the National Association of Social Workers. In addition, Ms. St. Germain facilitates a Caregiver Support Group at the Atrium at Faxon Woods in Quincy, an assisted living facility for people with Alzheimer's disease and dementia.
A native of Lakeville, Ms. St. Germain resides on the South Shore.
