As they advance to Level 3 certification for Patient- Centered Medical Home (PCMH) this July, CVFP is using i2iTracks to improve coordination of care, streamline care team workflows, and improve outcomes of care across their 11 practice sites and in collaboration with local medical facilities and providers.

    As one of the founding partners of the Collaborative Health Partners of Virginia an integrated network of primary care physicians, including Medical Associates of Central Virginia - CVFP is working toward improving and enhancing healthcare delivery throughout the region to deliver the highest quality care at the lowest possible cost. The efficient 70-day deployment of i2iTracks has already resulted in care team efficiencies, improved EHR documentation and proactive care outreach.     One of CVFP's top priorities is the redesign of our care teams including the addition of health navigators that coordinate care and improve patient engagement. "We wanted our health navigators to have the right tools to proactively manage our patient's health and to engage in outreach for important preventive care, i2iTracks will be critical in supporting the health navigators." said Crawford.

Through the use of i2iTracks, CVFP has improved EHR documentation and now has reports with more accurate data to support care teams in coordination of care, including lab and referral tracking, management of subpopulations, and tracking hospital admissions and discharges.

    As CVFP works toward PCMH transformation, i2iTracks has provided a systematic approach for implementing policies and protocols, reporting on clinical quality measures (CQMs), and transforming care teams to help their first site achieve Level 3 PCMH certified this July. "Without i2iTracks, we would have to do all our reporting and data capture manually, which would require more resources that we simply do not have," said Crawford. "i2iTracks has really helped us expedite the process." Their goal is to have their remaining 10 sites PCMH certified by the end of 2014.
